IF FOR ANY REASON YOU DENY SESSIONS OUTSIDE OF CENTER, YOU UNDERSTAND THAT ATS DOES NOT HAVE TO FILL THE HOURS YOU HAVE REQUESTED. EVERY EFFORT WILL BE MADE TO GET YOU THE AMOUNT OF HOURS YOU HAVE REQUESTED BUT IF IT IS NOT AVAILABLE WITH ONLY CENTER CLIENTS THEN WE CANNOT GUARANTEE THIS.

If there are any changes, you must submit a new form with at least 2 weeks in advance to review schedule request adjustment. (Schedule adjustments are not guaranteed)
